Q. How do I find a primary care physician?

Q. I was born at Swift County Benson Hospital. How do I obtain a copy of my birth certificate?
A. You may request your birth certificate from the town or city in which you lived when you were born. Or, you may request your birth certificate from the town or city where the hospital in which you were born was located. For those born at Swift County Benson Hospital, birth records are kept at Benson City Hall.
Q. How do I request a copy of my medical records, x-rays and/or mammogram films?

Q. How do I schedule a mammogram?
A. Please call us at (320) 843-4232 or (800) 324-0787 to request an appointment for a mammogram. You will need to provide information including your name, date of birth, phone number, address, and the name of the physician to whom we should send the report. If you have had films taken previously, you will be asked to bring those to your appointment. If you do not have insurance, you may be eligible for the Minnesota Sage Program.
