“Home Care for All Ages”

Who we are

Swift County-Benson Home Health Care is a Medicare-Certified agency that has served clients within 20 miles of Benson since 1993. As a Medicare/Medicaid home health agency we must be licensed and follow Federal healthcare guidelines.

Our goal is to have open communication with you, your physician and all the members of your health care team. We will work with your physician to implement a plan of care that will help you achieve your recovery goals. As a team we will regularly assess your progress and adjust your care plan so you always know what kind of progress you have made and what progress you can strive toward.

Who we serve

Our clients are people with chronic physical and mental disabilities, and medical and surgical patients recently discharged from a hospital needing home care services. We also provide respite care and adult and pediatric IV therapy.

We are proud of our professional staff

We have a staff of highly-skilled professional medical personnel including: registered nurses, licensed practical nurses, physical, occupational, and speech therapists, home health aides and homemakers. Each member of our staff attends at least twelve hours of advanced training annually to remain certified and licensed in their specialty. We also require criminal background checks for each person. As a team, we have the education and experience that allows us to easily coordinate your care plan with your physician and to give you the personal care that is so important to your recovery and comfort.

Services we provide

Some of our home health services include: wound care, IV therapy, daily in-home tele-monitoring of clients, maintenance of skilled pro-time anticoagulation checks, regular skilled nursing care for chronic health conditions, respite care, private pay bath or medication management program, and foot care clinics twice a month.

Skilled Nursing Care

Skilled Nursing Care: After hospitalization or surgery, your physician will often order help with medication instruction and health education. Many times skilled observation and assessments of your health status, medication management and wound care are ordered to help you remain in your home.

Rehab Therapy at Home: We offer physical, occupational and speech therapy.
Your doctor will decide which of these services you might need and recommend the number of visits. The visits will be written into your care plan and re-evaluated to determine if you need more time with the therapists.

  • Physical Therapy includes exercises, training to use adaptive/assistive devices, and ambulation training.
  • Occupational Therapy addresses approaches for self-help care. Our occupational therapists can assess your home for easier living, and design, fabricate and fit adaptive/assistive devices.
  • Speech Therapy provides in-home help for speech and swallowing difficulties.

Home Health Aides: Aides will assist you with daily living activities, personal hygiene and specific exercises. HHA’s services may be covered when employed by a Medicare/Medicaid certified home health agency such as Swift County-Benson Home Health.

Homemakers: When housekeeping and laundry services are needed, our homemakers provide private pay homemaking services with support and encouragement to persons living alone.

Diabetes Education Program: Whether you are a newly diagnosed diabetic or diabetic needing long term follow-up, our certified diabetes eduction program is tailored to meet your individual needs. Our program is highly flexile and individualized. Once we receive a referral from your provider, we will work closely with you to develop an exercise and food management plan that fits your lifestyle. The diabetes education offices are located on the second floor at SCBHS. For more information, please call 320-843-1349.

Foot Care Clinic

Foot Care Clinic: Having trouble with cutting your toenails and caring for your feet? Foot clinic is offered two times each month to anyone in the community who would like to schedule an appointment. Toenail and foot care is provided by one of our home health nurses. Foot clinic visits are not covered by insurance and the minimal cost is paid privately by each individual. This is a very popular community service so call early to set up an appointment time. For questions or appointment times, call 320-843-1349.

At Home Respite Care: Respite care allows caregivers to be away from home knowing their loved one is being cared for in their most familiar environment – home. Members of our professional staff will care for your loved for an hour, half day or a full day. Sometimes respite care is covered by long term care insurance, VA or other programs. Most families pay privately for this service. For more information please call us at 320-843-1349 to discuss how we can help you. Many times it is recommended by your doctor, but can also be suggested by your family.
