Chapel
The hospital chapel is located near the visitor entrance.
It is nondenominational and open to all patients, family
members and visitors around the clock. If you are admitted
and wish to see your spiritual advisor, you or a family
member must notify them to let them know you have been hospitalized. |

Check-in
Registration
All patients needing services will be required to register
in the outpatient waiting room. Please have all current
insurance information with you at the time of registration. |

Food
Services
Patient meals are served at 7:30 a.m. 11:30a.m. and 5:00
p.m. In keeping with the hospitals commitment to good health,
we prepare our meals using the dietary guidelines to increase
fiber and reduce fat, cholesterol, sodium and sugar. Your
preferences or substitutes may be relayed to the Dietary
department through the Dietary Supervisor or the nursing
staff. |

Front
Lobby Information Desk
Our front lobby information desk personnel will work
with you to help answer any questions or concerns
you might have. They may be required to put you in
contact with special hospital staff to answer your

| General
Visiting Policy
Our visiting hours are from 11:00am to 8:30pm. These hours
apply to both general visiting as well as obstetrics.
Visitors will be asked to leave during those times when
nursing care must be given to the patient. We would ask
visitors not to come at mealtimes if possible, unless
a relative or friend is coming to help feed the patient.
Restrictions
to visitation will apply as follows:
- Patient requests no visitors
- Physician writes an order restricting visitation
- During periods of flu outbreaks
The
SCBH has no restrictions on children visiting however
- All children must be attended by an adult visitor at
all times
- Children who are ill should not be brought in to visit
hospital patients
- Children must wear shoes at all times
Nursing
personnel may limit the number of visitors and duration
of the visit in the best interest of all patients. |

| Health
Insurance
Please bring your insurance policy identification and
numbers with you when you are admitted. If the information
is not available at that time, please call or send it
to the billing office as soon as possible. We will bill
your insurance carrier as a courtesy to you provided we
are given the necessary information. If
your insurance company requires prior authorization, please
let us know so we can contact your insurance company for
you. |

Home
Care
SCBH-Home Health Care is a hospital based Medicare Certified
Home Care program that provides continuity of care for patients
in their homes. Under the orders of a physician, the staff
of professional nurses are able to provide skilled services
in the home including IV, Physical, Occupational, and Speech
therapy as well as wound care, medication regulation and
health problem assessment. Home health aides and Homemakers
are also available to help people meet their personal care

Infant
Car Seat Rental
As each new arrival is ready to leave the hospital, they
are required to use a child safety seat. If you have not
yet purchased an infant safety seat, you may rent one from
us. For more information, please ask the charge nurse during
your stay. |

Lost
& Found
If you have lost an item while your were at the hospital,
please contact the front lobby information desk for assistance
in finding it. |

Patients
Rights
Patients will, at admission, be told that there are legal
rights for their protection during their stay at this facility.
You will receive a copy of the Patients' Bill of Rights
on admission. Please read through the document carefully.
If you have any questions or concerns about this information,

Rooms/Privacy
Each of our semi-private rooms includes a television, telephone
and bathroom for your convenience. |

SCBH
Auxiliary
The SCBH Auxiliary is active in helping maintain
and improve the hospital's equipment and facilities. Annual
activities throughout the year include educating the public
and sponsoring hospital fund-raising activities. For more

Smoking
Smoking is prohibited in the hospital at all times for the
health and safety of our patients and as mandated by Minnesota
law. |

Our team of doctors, nurse practitioners, nurses and support
staff are committed to treating each of our patients with
compassion and respect. We are here to make your stay with
us as comfortable as possible. Please do not hesitate to

Support
& Education Groups
The Swift County Benson Hospital provides many support and
education groups including Cardiac, Diabetes, Fibromyalgia,
Reach to Recovery (Breast Cancer, Post Mastectomy) and Pulmonary/Respiratory

Telephone
Each patient room has a telephone for your convenience.
You must dial 9 to get an outside line. All local calls
are free of charge and any long distance calls must be charged
to a phone card or made as a "collect" call. If you should
require assistance, dial "0" and ask for help from the information
desk. Additional phones are located in the front lobby and
hallway. |

Waiting
Rooms
We have several waiting rooms located throughout
the hospital. All waiting rooms are available
to visitors, family members and patients. Please
make yourself comfortable in these areas while
you are here. |
