Your right to make health care decisions under the law in Minnesota.
If you are 18 years or older, you have the right to make your own health care decisions, including the right to decide what medical care or treatment to accept, reject or discontinue. You may wish to name someone to make health care decisions for you. You have the right to make these desires known to your doctor, hospital or other health care providers to make sure your rights are respected. In the event that you cannot speak for yourself, Advance Directives can provide you a way to make your specific wishes about the medical care you want to receive known to your health care providers.
If you have questions please call Michele Sonnabend, Discharge Planner at SCBHS — 320-843-1377
