You may request your birth certificate from the town or city in which you lived when you were born. Or, you may request your birth certificate from the town or city where the hospital in which you were born was located. For those born at Swift County Benson Health Services, birth records are kept at Benson City Hall.

  • To schedule a physical or mammogram: Call (320) 843-2030
  • To schedule a mammogram only: Call (320) 314-1551
    • You will need to provide information including your name, date of birth, and name of your primary health care physician.
    • You will also have to make sure it has been one year since your previous mammogram and you are not noticing any pain or lumps.
